All other licenses require applications which are not available via this web site. To receive the appropriate application you may contact the Board office or submit the Social Work License Application Request Form (below) by fax or mail.

If you call the Board office for an application, we will require all information that you see on the application request form below. You may also download a printable copy of the application request form below.
