Introduction Indiv. Approved Certified Providers 6-Month Activity Rpt Applications Policies & Procedures

Certified Provider Six Month Activity Report:

Certified Providers must complete and submit the Certified Provider Six Month Activity Report each six months detailing program activity within the State of WV.

This report is required to be submitted by:

  • January 15th for programs held between July 1 and December 31 of the preceding calendar year - OR -
  • July 15th for programs held between January 1 and June 30 of the current year.
